• There will be TWO BOOSTER VACCINATION dates – January 22 and February 5. If you cant make your schedule – either you are sick and want to transfer from one date to another, please message Dr. Bogs Rivera at this email… lifechangingweb@gmail.com
   
• OTHER REGISTRANTS have duplicate entries. They have entries on every both scheduled dates. Please let me know which day you prefer. This would pave more individuals to receive vaccines by vacating your reservation.
• Bring your FACESHIELD, and FACE MASK, preferably K 95, KN95, KF94, or use double mask – cloth mask over a 3-ply surgical mask
